Answer : 3 all sarcoidosis



 



Chest CT scan of case 1 patient shows
typical pattern of pulmonary sarcoidosis with bronchovascular bundle thickening
and irregular nodular lesions


 



 Case2 - No invovement of sarcoidosis in other organs except spleen

         - Only spleen involvemen of sarcoidosis can happen.


  Case 3 - Initially on spleen was involved and splenectomy done, On F/U chest invovement of sarcoidosis was confrimed. 

          




Sarcoidosis in abdomen



-      
Frequently involved , although
usually asymptomatic(>95%)



-      
Liver and spleen ( 40-60%),
kidney ( 8-20%), pancreas, intestine, testis(<5%)



     

Liver and spleen involvement

Alkaline phosphatase elevation



-      
Fever, malaise, weight loss



-      
Organomegaly with homogeneous
attenuation; most common finding.



-      
Focal nodules ( 5- 25%) R11;
Coalescing granulomas



-      
Innumerable, variable size,
diffuse distribution,



-      
CT : hypodense nodules



-      
MRI : Low SI on all sequences,



     Hypoenhance relative to
background liver



-      
US : hypo/hyperechoic nodules,
depend on echogenicity of liver or degree of fibrosis present in granuloma



 

Abdominal lymphadenopathy



-      
Moderate lymphadenopathy ( 30%)
: about 1cm or less



-      
Extensive lymphadenopathy (
10%) : > 2cm, and involvement of > 4 sites



 

Differential diagnosis



-      
Metastasis : uncommon simultaneous
involvement of both liver and spleen than sarcoidosis or lymphoma



-      
Lymphoma : more prominent and
frequent conglomerate of nodes



            Retrocrural involvement



 



-      
Tuberculosis : usually
association with miliary pulmonary involvement
