| Comments | •Extrahepatic bile duct, segmental resection with cholecystectomy: Adenocarcinoma, moderately differentiated, common hepatic and common bile ducts 1) Size of tumor: about 1.7x1.7x0.8cm 2) Depth of invasion: 0.8cm 3) Extent of invasion: perifibromuscular connective tissue 4) Invasion of cystic duct: present 5) No invasion to gallbladder 6) Blood vessel invasion: absent 7) Lymphatic invasion: absent 8) Perineural invasion: present 9) Surgical margins: clear (safety margin: 0.6cm from proximal duct margin, 1.1cm from distal margin) 10) Lymph node: metastasis in one out of 8 lymph nodes (pericholedochal LN: 0/2, LN #12: 0/5, LN #16 (Fro 1): 1/1) 11) Pathologic stage: pT2N1 (AJCC 2017), pT2N1 (AJCC 2010) 12) Related biopsy: P17-16912, S17-22652 13) Associated findings: Bil IN, high grade/low grade, common bile duct * 임상적 의의 AGC로 follow up 하는 환자에서 새롭게 발생한 bile duct wall thickening에 대해 관성적으로 peribiliary mets.로 판독하는 경향이 있으나, NED state의 고령의 환자에서 새롭게 발병한 double primary cholangioca.의 가능성의 염두에 두고 수술 가능성을 재고해야함. 이번 case의 경우 1) 1년전 CT에서 short segmental stricture, CTx. 없이 비교적 slow growing 2) 금번 CT check 한달 후 MRCP에서 CHD에 segmental luminal narrowing 3) pathology (첨부한 mapping 참조) : 주변부에 Bil IN, dysplasia가 있어서 이전 AGC pathology와 비교 없이도 cholangioca.로 보는 것이 타당하다고 함. 육안 소견도 ovoid한 형태의 1.7cm size 작은 mass.
* limitation 및 출제 의도 제한된 외부병원 CT, MR 영상으로, cholangitis가 동반되어 ductal wall thickening의 범위가 과장되어 보이며 diffuse wall thickening type의 cholangiocarcinoma와 peribiliary mets.가 사실상 감별이 어려우나 위와 같은 임상정보가 주어졌을때 double primary ca. 가능성을 열어 두어야 한다는 교훈적 case로 출제함. 실제 weekly conference에서 image review나 임상정보 없이 출제자가 받은 질문에 임상정보 및 이전 영상을 추가해 퀴즈로 구성함. |
